ILSM may collect personal information such as name, address, phone number, email address, date of birth, and health information in order to provide services to our members. ILSM will only collect, use, and disclose personal information for the purposes that have been identified to you, unless we have obtained your consent to use it for another purpose. We will obtain consent from individuals before collecting their personal information. ILSM will not sell, rent, or trade personal information to third parties.
ILSM takes reasonable measures to protect personal information from unauthorized access, use, or disclosure. We have implemented physical, technical, and administrative safeguards to protect personal information from loss, theft, and misuse.
ILSM retains personal information only as long as necessary to fulfill the purposes for which it was collected, or as required by law. We take reasonable steps to destroy or de-identify personal information when it is no longer required.
Independent Living Sudbury Manitoulin does not disclose personal information to third parties except in the following circumstances:
We may disclose personal information to third parties with your consent. For example, if you apply for a service that requires the involvement of another organization or individual, we may need to share your personal information with them to complete the service.
We may disclose personal information to third parties if we are required to do so by law or if we believe that such disclosure is necessary to comply with a legal process, such as a court order.
We may disclose personal information to third parties if we believe it is necessary to investigate, prevent, or take action regarding suspected illegal activities, fraud, threats to our organization, or violations of our policies and procedures.
Individuals have the right to access their personal information held by ILSM. To request access to personal information, please contact our Executive Director. ILSM takes reasonable steps to ensure that personal information is accurate, complete, and up-to-date. Individuals are responsible for informing us of any changes to their personal information.
Individuals who have concerns about the way ILSM handles their personal information may contact our Executive Director to discuss their concerns. We will investigate all complaints and take appropriate measures to address any privacy concerns.
